             •   Who you are as a person,

             •   A statement about your hearing loss,
 
             •   How you prefer to communicate, and

             •   How you use technology to assist yourself

After you complete the form, it is recommended you share the letter with your teachers, friends, employer, college, or anyone else who may interact with you. Feel free to add any other important information about yourself you would want others to know in order to help communicate more effectively. Use the opportunity to have a conversation with the person to help them better understand communicating with people who are deaf or hard of hearing.
