GAP Transition Checklist covers twelve important areas that lead to self-advocacy and personal responsibility. These areas are: Hearing Loss and Hearing Technology, Communication and Accommodations, Social Skills, Functional Skills, Money Management, Survivor Skills, Work-Related Skills, Communication Skills, School/Work Related Grooming Skills, Health Skills and Safety, Recreational Skills, and Education/Training After High School. You will probably want the assistance of your teacher, counselor or parent when completing this checklist. Following completion of the checklist, you should identify your personal learning goals into GAP Transition Planner.
